As a Band 5 Locum Community Nurse, you will provide high-quality nursing care to patients in their own homes or community settings. You will work closely with multidisciplinary teams to support patients with a range of health conditions, promoting independence and improving their quality of life.

Key Responsibilities

• Deliver nursing care in patients' homes, clinics, or other community settings.
• Manage wound care, including dressings, pressure ulcer care, and infection prevention.
• Administer medications, including injections, and monitor patient responses.
• Provide palliative and end-of-life care with sensitivity and professionalism.
• Assess and manage patients with chronic health conditions, such as diabetes or respiratory illnesses.
• Perform clinical observations, including blood pressure, pulse, and temperature monitoring.
• Educate patients and families on health promotion, self-care, and managing long-term conditions.
• Maintain accurate and timely patient records, adhering to professional and organizational standards.
• Work collaboratively with other healthcare professionals, including GPs, physiotherapists, and social care teams.
• Ensure all care is delivered in line with relevant policies, guidelines, and infection control procedures.

Essential Requirements

• NMC registered nurse with a valid PIN.
• Previous experience working as a Community Nurse or in a similar role.
• Competent in clinical skills such as wound care, catheterization, and medication administration.
• Excellent communication and interpersonal skills, with a compassionate and patient-centered approach.
• Ability to work independently and manage a caseload effectively.
• A full UK driving license and access to a vehicle.
