Description

We are looking for a dedicated and skilled Locum District Nurse. This position is ideal for an experienced nurse who thrives in a community setting and is committed to providing high-quality, patient-centered care. The successful candidate will deliver nursing care in patients' homes and other community settings, supporting patients to live independently and manage their health conditions effectively.

Key Responsibilities:

  • Conduct comprehensive assessments of patients in their homes or community settings.
  • Develop and implement individualized care plans in collaboration with patients, their families, and other healthcare professionals.
  • Provide wound care, medication administration, chronic disease management, palliative care, and other nursing interventions.
  • Monitor patients' conditions, identify changes, and respond appropriately to ensure optimal care.
  • Educate patients and their families on managing health conditions and promoting self-care.
  • Collaborate with GPs, social services, and other healthcare professionals to coordinate care and support services.
  • Maintain accurate and detailed patient records in accordance with NHS guidelines.
  • Participate in case conferences and multidisciplinary team meetings.
  • Provide mentorship and support to junior nurses and healthcare assistants.
  • Engage in clinical audits, research, and continuous quality improvement initiatives.

Essential Criteria:

  • NMC registration as a Registered Nurse.
  • Extensive experience in community nursing or district nursing.
  • Strong clinical skills in assessment, planning, and delivery of nursing care in a community setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to ongoing professional development and evidence-based practice.
  • Experience in working within an NHS or similar healthcare setting.
