We are currently seeking a motivated and skilled Locum Community Dietitian. This role offers an exciting opportunity to work within a dynamic team and make a real difference in the nutritional health and well-being of patients in the community.

Key Responsibilities:

Conduct nutritional assessments and provide individualized dietetic advice for a range of conditions, including diabetes, malnutrition, gastrointestinal disorders, and other chronic health conditions
Work with patients in various community settings, including homes, clinics, and care facilities, ensuring the highest standard of care
Develop and implement care plans, providing tailored nutritional interventions and ongoing support to patients and caregivers
Collaborate with multidisciplinary teams, including doctors, nurses, physiotherapists, and social workers, to provide holistic care
Educate and support patients and their families on dietary management and healthy lifestyle choices
Monitor patient progress and adjust care plans as needed to ensure the best possible outcomes
Participate in team meetings, case conferences, and service reviews, contributing to the ongoing development of the community dietetics service
Maintain accurate and up-to-date records in line with organizational and professional guidelines
Promote health and well-being through nutrition education and preventative initiatives in the community

Requirements:

Degree in Dietetics or equivalent, with HCPC registration as a Dietitian
Experience in providing dietetic services within a community or outpatient setting
Strong clinical skills in nutritional assessment, care planning, and delivering evidence-based dietary interventions
Excellent communication and interpersonal skills, with the ability to build rapport with patients and work effectively within a multidisciplinary team
Ability to manage a varied caseload and prioritize work effectively
Full driving license and access to a vehicle (if community visits are required)
