Description
We are currently seeking a motivated and skilled Locum Community Dietitian. This role offers an exciting opportunity to work within a dynamic team and make a real difference in the nutritional health and well-being of patients in the community.
Key Responsibilities:
- Conduct nutritional assessments and provide individualized dietetic advice for a range of conditions, including diabetes, malnutrition, gastrointestinal disorders, and other chronic health conditions
- Work with patients in various community settings, including homes, clinics, and care facilities, ensuring the highest standard of care
- Develop and implement care plans, providing tailored nutritional interventions and ongoing support to patients and caregivers
- Collaborate with multidisciplinary teams, including doctors, nurses, physiotherapists, and social workers, to provide holistic care
- Educate and support patients and their families on dietary management and healthy lifestyle choices
- Monitor patient progress and adjust care plans as needed to ensure the best possible outcomes
- Participate in team meetings, case conferences, and service reviews, contributing to the ongoing development of the community dietetics service
- Maintain accurate and up-to-date records in line with organizational and professional guidelines
- Promote health and well-being through nutrition education and preventative initiatives in the community
Requirements:
- Degree in Dietetics or equivalent, with HCPC registration as a Dietitian
- Experience in providing dietetic services within a community or outpatient setting
- Strong clinical skills in nutritional assessment, care planning, and delivering evidence-based dietary interventions
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and work effectively within a multidisciplinary team
- Ability to manage a varied caseload and prioritize work effectively
- Full driving license and access to a vehicle (if community visits are required)
