We are currently seeking a Locum Band 5 District Nurse to join our dedicated team. This is an exciting opportunity to provide patient-centered care in a dynamic and supportive community setting.
Job Overview:
As a Locum Band 5 District Nurse, you will play a crucial role in delivering nursing care to patients in their homes and community settings. You will work as part of a multidisciplinary team to assess, plan, and deliver care to patients with a wide range of health conditions, promoting independence and improving quality of life.
Key Responsibilities:
- Provide high-quality nursing care to patients in their homes or community settings.
- Assess, plan, implement, and evaluate individual patient care plans.
- Perform clinical tasks such as wound care, administering medications, and monitoring long-term conditions.
- Work closely with GPs, other healthcare professionals, and family members to coordinate patient care.
- Educate and advise patients and carers on managing conditions and promoting health and well-being.
- Maintain accurate and up-to-date patient records.
- Support the team with managing caseloads effectively and meeting patients' individual needs.
- Contribute to the safeguarding and well-being of vulnerable individuals in the community.
- Actively participate in clinical supervision, audits, and quality improvement initiatives.
Qualifications and Requirements:
- Registered Nurse with valid NMC registration.
- Minimum of 1-year post-registration experience, preferably in a community or district nursing setting.
- Competency in performing nursing assessments, care planning, and delivering care in a community setting.
- Full UK driving license and access to a vehicle for community visits.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Compassionate, patient-focused, and adaptable to the changing needs of the community.
