Description
We are having an excellent opportunity for Advanced Nurse Practitioner for London location.
Purpose:
- To provide high quality, person-centred, holistic nursing care to patients registered with or seeking care at our Healthcare practices, including screening, assessment and treatment
- To see patients at first contact for undifferentiated problems and for follow-up, managing their presenting symptoms, minor self-limiting conditions and long-term conditions, and deal with their physical, psychological and social problems
- To promote and facilitate a healthy lifestyle and to provide preventative health care and health education
Responsibilities:
- To assess and diagnose across a wide range of primary health care conditions, order investigations, plan and provide treatment as needed and discharge patients without reference to a doctor
- To assess, tolerate and manage risk and deal with uncertainty
- To prescribe in accordance with regulations and with the post holders training and registration
- To initiate appropriate and timely consultation and/or referral when a presenting condition exceeds the nurse practitioner’s scope of practice and/or expertise
- To liaise with a doctor about patients with complex problems who need the expertise of a general practitioner
- To provide, develop and evaluate health promotion and disease prevention services to patients, including those who are healthy and those who have acute and/or long-term conditions
- To undertake screening procedures
- To ensure that appropriate health promotion materials are provided effectively
- To work collaboratively to develop and implement a predominantly nurse-provided model of care for patients with primary health or social care needs
- To be an expert resource for and to undertake the appropriate management of long-term conditions and the diagnosis and treatment of primary care conditions
- To assist and advise on the development of a model of primary care triage
- To provide telephone consultations as required
- To perform house visits for house bound patients with undifferentiated diagnosis.
- To use and develop protocols and guidelines for the delivery of care to patients with primary health care needs
According to the needs of the patient, to assess, plan, implement, monitor and evaluate the primary care services provided - To work collaboratively to identify the skill mix required to meet the health and social care needs of patients
