We are looking for healthcare assistants who will liaise at all levels with consultants, nursing services managers and all members of our multi-disciplinary team.

Responsibilities:

• Assisting members of the multi-disciplinary team in the assessment, planning, implementation and evaluation of care for patients.
• Participating in the review of patients’ care plans as appropriate in ward rounds, meetings and case conferences, in line with policies and procedures.
• Reporting and recording any changes in a patient's physical or mental condition to the nurse in charge.
• Liaising with other professionals and agencies as required.
• Escorting patients both internally and externally to the hospital, as required.
• Endeavouring to develop sound professional relationships with patients, to promote active listening and communicate the patient's needs to members of the multi-disciplinary team.
• Ensuring that confidentiality is maintained at all times in accordance with the company policies, the Data Protection Act 1998 and the Health Records Act 1990.
